You need a plan.

Why choose a Life Care Plan?

• Skip the time, cost, and stress of court proceedings for your loved ones

• Keep things private

• Have a plan to stay in control if you or your spouse becomes ill or incapacitated

Update your trust as your life, assets, or
family situation changes

Provide clear instructions and protect beneficiaries

You need to get an estate plan you can trust,
with someone to guide you every step of the way.

If something happened today, are you prepared?

Time is limited

When crisis strikes, it's too late

We can't go back and make a plan after the fact. No one wants to think about a worst case scenario, but if disaster does strike you or your family, it is often too late to put the legal documents in place that could fully protect you.

a plan for peace of mind

Your legal plan should reflect where you are

Are you at a stage of life where you have something to worry about? Having a Life Care Plan benefits those who are experiencing a health decline or have a loved one who has. You want the guidance of someone to navigate you through what to line up to protect you both.

options to change

A versatile plan for the future

What you need today will change in the future. Estate plans aren't forever, they need to change when your life changes. A Life Care Plan can be converted to a Medicaid Asset Protection Trust. Options for whatever life may bring.

the solution

The Life Care Plan

Solving the legal, financial, and care needs of aging adults.

Meet Lisa

After more than a decade guiding families through estate planning, trusts, Medicaid strategies, and elder law, one truth stands out: preparing for a possible health decline—whether for yourself or a spouse—requires more than legal documents. It takes a clear plan and someone to walk with you through the process.

That’s why our approach includes not just legal strategy, but also the support of an Elder Care Coordinator. With their guidance, you’ll have a knowledgeable partner to help you navigate assessments, educate you on your options, coordinate needed care, and advocate for your goals. Whether you’re anticipating the slow progression of aging or planning ahead for a known condition, this is about staying ahead—not reacting in crisis.

The Life Care Plan brings together customized legal tools and compassionate, expert guidance—so you’re not just prepared legally, but supported personally. It’s a roadmap for the future, built with care and clarity, so you and your loved ones can move forward with confidence.

SPECIAL OFFER

INTRODUCING

The Life Care Plan

Get peace of mind with the Life Care Plan— A holistic, elder-centered approach to planning that helps families navigate the journey through the long term care maze by offering customized solutions for the financial, legal, and personal care needs of aging and disabled family members.

WHAT'S INCLUDED

A real attorney!

Online form websites are required by law to disclose they are NOT giving you legal advice. That is the OPPOSITE of the Essentials Plan. You will meet with an attorney and have your information reviewed to ensure you are getting what you actually need.

A financial power of attorney

If you are unable to manage your finances and don't have a financial power of attorney signed, your family will need to petition the Probate Court to be appointed as Conservator. This long and costly process can be avoided with this important directive. Name an agent to take care of your finances, along with a back-up, in case you ever need it.

A testamentary will

This plan includes a revocable living trust that covers your wishes. You can choose who you want to distribute your assets when you pass away, where you want your assets to go, who you want to be in charge of your funeral plans, and if you have minor children, you can nominate guardians and set up a family trust for their future.

A testamentary will

This plan includes a pour-over will. If you forget (or don't get the chance) to transfer all of your assets into your trust during your lifetime, the pour-over will "catches" those leftover assets when you pass away—and directs them into your trust.

A health care power of attorney

If you don't have a health care power of attorney and you are too ill to make decisions for yourself, whether it's temporary or permanent, state law determines who makes those decisions unless you complete your own directive and decide what you want and who you want making those decisions regarding care.

A HIPAA authorization form

If you don't have a health care power of attorney and you are too ill to make decisions for yourself, whether it's temporary or permanent, state law determines who makes those decisions unless you complete your own directive and decide what you want and who you want making those decisions regarding care.

Asset titling instructions

A revocable trust only controls the assets that are actually put into it. Asset titling instructions tell you how to properly name (or “title”) your assets so that they are either

owned by the trust during your lifetime, or set up to transfer to the trust when you pass away.

A personal property memorandum

Do you want to give jewelry, handguns, coins, art, or other items to certain family or friends? This memorandum is the legal way to do it. Much better than a sticky note on everything!

Elder Care Coordination

Ongoing support from an Elder Care Coordinator who guides you through each stage of planning and transition—providing assessment, education, coordination, and advocacy every step of the way.

Comprehensive planning

A plan for if and when you need care so that if you lose capacity, someone else can step in seamlessly to support you in your goals, preferences, and wishes. This will include
access to yearly financial plan reviews, asset titling progress and a "GoBinder" to keep your planning documents organized.

Who can benefit from this planning

You must qualify for the Life Care Plan. This Plan is ideal particularly for those who are anticipating or experiencing health-related changes.

Legal Protection Shielding families from legal challenges with comprehensive planning, ensuring a secure foundation for the future

Financial Stability Strategizing to protect assets and maximize resources for the long term

Healthcare Support Tailoring plans to address current and future needs, ensuring quality care

Family Unity Returning caregivers back to being family members while we assist with the challenge of aging and caregiving

A final thought

No one ever says, "I wish my loved one wouldn't have planned ahead."

But family members are often disheartened to learn that their loved one did not take steps to get the right legal documents, create a plan, or leave guidance for their family.

This is your chance to take action today to make sure your family doesn't have this experience. The time is now.

But I have questions...

What is the total cost?

Our clients routinely hire us in order to preserve and protect significant assets. If your loved one needs nursing home level care at $10,000 a month and you wait 3 months to meet with us, you may have already spent $30,000 that could have been part of an asset protection plan. The cost of private paying full price for skilled care will almost always significantly outweigh our fee for services. For many clients, it ends up being a fraction of the income and assets they are able to save. Are there cheaper options? Of course there are cheaper options. Our philosophy is that you get what you pay for and plan for. We all know this to be true. Our skilled team navigates complex government programs, advises our clients in a tax efficient way, provides strategy for various issues, and provides comprehensive services that align legal, financial, tax, and health care needs.

What do I need to do before my consultation?

After you pay your deposit and schedule your consultation, you will receive a link to log into our online questionnaire. It is very important you complete this prior to your consultation. If you cannot complete this, please reschedule your consultation time.
